A reside-action film primarily based on the manga was released in Japan on July 29, 2017. An anime adaptation based on the sequel manga Tokyo Ghoul:re started airing on April 3, 2018, with the series getting split into two seasons.

Tokyo Ghoul is set in an alternate reality where ghouls, cannibalistic 'monsters' who look exactly like standard people but can only survive by consuming human flesh, live amongst the human population in secret, hiding their true nature in order to evade pursuit from the authorities. Such as enhanced speed, senses, and regenerative capability, a normal ghoul is many instances stronger than a standard human, has a skin resistant to ordinary piercing weapons and has at least one unique predatory organ called a kagune (Japanese: 赫子), which it can manifest and use as a weapon in the course of combat. Yet another distinctive trait of ghouls is that when they are excited or hungry, the colour of their sclera in both eyes turns black and their irises red. This mutation is recognized as kakugan (赫眼, "red eye").

A half-ghoul can either be born naturally as a ghoul and a human's offspring, or artificially developed by transplanting some ghoul organs into a human. In each circumstances, a half-ghoul is normally considerably stronger than a pure-blood ghoul. In the case of a half-ghoul, only 1 of the eyes undergoes the "red eye" transformation.
