A live-action film primarily based on the manga was released in Japan on July 29, 2017. An anime adaptation primarily based on the sequel manga Tokyo Ghoul:re started airing on April three, 2018, with the series becoming split into two seasons.

Tokyo Ghoul is set in an alternate reality where ghouls, cannibalistic 'monsters' who look specifically like typical men and women but can only survive by eating human flesh, live among the human population in secret, hiding their accurate nature in order to evade pursuit from the authorities. Including enhanced speed, senses, and regenerative capability, a regular ghoul is a number of instances stronger than a typical human, has a skin resistant to ordinary piercing weapons and has at least a single specific predatory organ known as a kagune (Japanese: 赫子), which it can manifest and use as a weapon for the duration of combat. One more distinctive trait of ghouls is that when they are excited or hungry, the color of their sclera in each eyes turns black and their irises red. This mutation is identified as kakugan (赫眼, "red eye").

A half-ghoul can either be born naturally as a ghoul and a human's offspring, or artificially created by transplanting some ghoul organs into a human. In each circumstances, a half-ghoul is generally significantly stronger than a pure-blood ghoul. In the case of a half-ghoul, only a single of the eyes undergoes the "red eye" transformation.
