A live-action film primarily based on the manga was released in Japan on July 29, 2017. An anime adaptation based on the sequel manga Tokyo Ghoul:re began airing on April 3, 2018, with the series becoming split into two seasons.

Tokyo Ghoul is set in an alternate reality where ghouls, cannibalistic 'monsters' who appear precisely like typical people but can only survive by consuming human flesh, reside amongst the human population in secret, hiding their accurate nature in order to evade pursuit from the authorities. Such as enhanced speed, senses, and regenerative potential, a regular ghoul is several occasions stronger than a regular human, has a skin resistant to ordinary piercing weapons and has at least one specific predatory organ known as a kagune (Japanese: 赫子), which it can manifest and use as a weapon for the duration of combat. Another distinctive trait of ghouls is that when they are excited or hungry, the color of their sclera in both eyes turns black and their irises red. This mutation is known as kakugan (赫眼, "red eye").

A half-ghoul can either be born naturally as a ghoul and a human's offspring, or artificially designed by transplanting some ghoul organs into a human. In each instances, a half-ghoul is normally much stronger than a pure-blood ghoul. In the case of a half-ghoul, only one particular of the eyes undergoes the "red eye" transformation.

There is also the case of half-humans, hybrids of ghouls and humans that can feed like regular humans and lack a Kagune even though possessing enhanced abilities, like improved speed and reaction speed, but shortened lifespans. To hunt down the ghouls, numerous government-sponsored organizations such as the CCG had been designed worldwide.
