A live-action film based on the manga was released in Japan on July 29, 2017. An anime adaptation primarily based on the sequel manga Tokyo Ghoul:re began airing on April three, 2018, with the series becoming split into two seasons.

Tokyo Ghoul is set in an alternate reality where ghouls, cannibalistic 'monsters' who appear exactly like standard men and women but can only survive by eating human flesh, reside among the human population in secret, hiding their correct nature in order to evade pursuit from the authorities. Including enhanced speed, senses, and regenerative potential, a normal ghoul is numerous instances stronger than a regular human, has a skin resistant to ordinary piercing weapons and has at least a single special predatory organ referred to as a kagune (Japanese: 赫子), which it can manifest and use as a weapon throughout combat. Yet another distinctive trait of ghouls is that when they are excited or hungry, the colour of their sclera in both eyes turns black and their irises red. This mutation is recognized as kakugan (赫眼, "red eye").

A half-ghoul can either be born naturally as a ghoul and a human's offspring, or artificially produced by transplanting some ghoul organs into a human. In each instances, a half-ghoul is normally a lot stronger than a pure-blood ghoul. In the case of a half-ghoul, only one of the eyes undergoes the "red eye" transformation.

There is also the case of half-humans, hybrids of ghouls and humans that can feed like normal humans and lack a Kagune although possessing enhanced skills, like increased speed and reaction speed, but shortened lifespans. To hunt down the ghouls, a number of government-sponsored organizations such as the CCG were produced worldwide.
