A live-action film based on the manga was released in Japan on July 29, 2017. An anime adaptation primarily based on the sequel manga Tokyo Ghoul:re started airing on April three, 2018, with the series being split into two seasons.

Tokyo Ghoul is set in an alternate reality exactly where ghouls, cannibalistic 'monsters' who look precisely like regular folks but can only survive by consuming human flesh, live amongst the human population in secret, hiding their correct nature in order to evade pursuit from the authorities. Such as enhanced speed, senses, and regenerative potential, a standard ghoul is a number of instances stronger than a typical human, has a skin resistant to ordinary piercing weapons and has at least one specific predatory organ named a kagune (Japanese: 赫子), which it can manifest and use as a weapon in the course of combat. Yet another distinctive trait of ghouls is that when they are excited or hungry, the color of their sclera in each eyes turns black and their irises red. This mutation is identified as kakugan (赫眼, "red eye").

A half-ghoul can either be born naturally as a ghoul and a human's offspring, or artificially designed by transplanting some ghoul organs into a human. In each instances, a half-ghoul is typically a lot stronger than a pure-blood ghoul. In the case of a half-ghoul, only a single of the eyes undergoes the "red eye" transformation.
